The Healthy Smiles includes a variety of dental plans to select from. - ACCESS PLANS
After selecting the type of coverage that you need, you will be asked to accept terms and conditions and then be asked to create a personal account. You will be required to setup a unique username and password to continue. From there follow the prompts to complete the application process. If you are unable to complete the application immediately, save your progress and complete the application later.
